[pjsip] video call

conf bridge/switch board is only used by audio.
There are video encoding/decoding clock thread polling video flow. And
video stream/video port objects are something like audio objects. Also
there are converter object inside video port for format/size converting
between video stream and video window.    As Bill said, video tee port is
connecting preview and encoding stream.
There are some synchronization code at pjmedia. but I didn't read into it.
